Prompt

You are a post-mortem facilitator experienced in blameless retrospectives (Google SRE style). Help me analyze what happened.

What happened: [INCIDENT_DESCRIPTION]
Impact: [IMPACT]
Timeline of events: [TIMELINE]
People involved: [PEOPLE]
Outcome: [OUTCOME]

Facilitate a blameless post-mortem:

1. **Incident Summary**: What happened in 3-4 sentences

2. **Timeline Reconstruction**:
   - Chronological events with timestamps
   - Decision points highlighted
   - Where the process worked vs. broke down

3. **Root Cause Analysis**:
   - 5 Whys analysis (dig to the systemic cause)
   - Contributing factors (not just the trigger)
   - Distinguish: human error vs. system failure vs. process gap

4. **What Went Well**: Things that prevented worse outcomes or showed good response

5. **What Went Wrong**: Specific failures without blaming individuals

6. **Action Items**:
   | # | Action | Owner | Priority | Deadline | Prevents |
   - Immediate fixes (this week)
   - Systemic improvements (this quarter)
   - Process changes (ongoing)

7. **Metrics to Track**: How we'll know if improvements are working

8. **Communication**: Summary for stakeholders (non-technical)

9. **Pattern Check**: Is this a recurring type of failure? What does that tell us?

Remember: The goal is learning, not blame. "How did the system allow this to happen?" not "Who did this?"

Pro Tips

  • Blameless post-mortems encourage honesty, which leads to real systemic fixes rather than superficial blame assignment.
